Venous thromboembolism develops when the fragile balance between blood coagulation and dissolution of the thrombus is impaired. Normally, once the injury is fixed, thrombus is dissolved. More than 20 different molecules participate in coagulation and fibrinolysis. The cascade itself is quite complicated and delicate process. There are plenty of chances for things to go wrong.

You don’t necessarily have to cut your finger or graze a knee, tiny molecules circulating in your blood are ready to gather and create protective cluster even when nothing bleeds externally. That is why those tine molecules responsible for creating thrombus occur on greater extent right there.

Thrombosis happens when a thrombus or its part breaks off and travels via the blood stream until it gets stuck. It can cause severe circulatory disorder. Depending on where a thrombus blocks the vein you can suffer from vein thrombosis, heart attack, stroke or pulmonary embolism. The condition of being predisposed to thrombus creation is called thrombophilia. It can be both inherited as well as acquired from bad health habits and external factors.

Inherited predisposition to thrombophilia means that there is a mutation or mutations in genes regulating blood coagulation and fibrinolysis:

  • Mutation in gene for Factor V, so called Leiden mutation. The most dangerous one.
  • Mutation in gene for prothrombin which is Factor II.
  • Mutation in gene MTHFR. This one is considered to be dangerous during pregnancy.

The severity depends on whether the mutation affects only 1 copy of the gene or both. Logically, mutations in both copies means greater risk of thrombosis.

Other risk factors that you can more or less influence and change:

  • Smoking
  • Obesity
  • Varicose veins
  • Immovability or low physical activity (leg in gypsum, long term sitting)
  • Hormonal contraception. Together with Leiden mutation it increases the risk of thrombosis 30 times.
  • Hormonal substitutional therapy during menopause
  • Pregnancy and/or childbed
  • Cancer
  • Chronic intestinal diseases
  • Sepsis
  • Autoimmune diseases (i.e. diabetes)

Many obese patients who are trying to handle their weight are being encouraged to undergo DNA tests for thrombophilia. But with a normal person, it is not that easy.
